Here are the steps to creating an op tin form using eResponder Pro
1. Make sure you have selected the right campaign name for the form you are creating
2. Click on forms from the tabs menu
3. Click on create form
4. Type the name of your form in the “Form Name” field
5. Select your thank you page or designate a custom thank you page URL
6. Type an identifier in the “Ad Tracking” field (optional)
7. Designate the fields you want your subscribers to enter in the form. You can select from Full Name, First Name, Last Name, and email address.

8. Enter button text
9. Click on the “Create” button to create the opt in form.
10. Click on the HTML button under the ACTION area
11. Copy and paste form on to webpage or in widget area of your blog.
